When is it acceptable to use "they" as opposed to "him/her"?

If you've ever read Poe's story "The Tell-Tale Heart", you might have noticed that the narrator doesn't have a gender. But it's not acceptable to refer to the narrator as a "they". So should I just call the narrator a "him"? Or "him/her"? Or should I just say "the narrator" as much as possible?
